If you die a lot, then you'll get Very Easy mode. I'm guessing that it decreases the damage of some attacks, makes your attacks more powerful, and also lessens the amount of enemies you encounter.

Do NOT save after you beat the water guardian. You will make that save unusable to play and go back to levels.

That's the ass fairy that shots you out of her ass. Just do not press any keys when you are in her ass.
The hidden scene is the shopkeeper Emily. Do not buy the same item multiple times (or try to buy an item you cannot afford).

Nah, you can play in English with the Japanese version. It's just an option in the settings menu.
