I've been wanting to do something like this for a while, but life has been horrendous to my work output. x_x

I have a few ideas for how this concept would work. My first instinct is to just make combat/events that remove a member of your party when they 'die' and flip an event switch that signals they are dead in further events. Could probably make a small town/dungeon based around the scenario if I felt up to it. Hell, I'm already thinking of ways to do it in a longer game but I already got way too many things I wanna work on.

I think the type of game you're looking for is called a roguelike. Here's two that I know of that are mods of existing good roguelikes:

There's also Nomad made by darkevilme, but it doesn't have the perma-death feature but is still a vore-focused roguelike:

I would just think that you'd just start the small than go bigger with each update. I guess the devs of Watch Dogs Legion and other games with multiple player characters also figured that such a mechanic is "b" to implement.

They're promising - but I'd prefer more if they were their own games rather than mods. I know that's picky of me to say, but it would be my preference. I do wonder - if such a game would be considered to be made - i wonder how many arms and legs it'll take? or if it'll cost an arm and leg at all.

I could think of some ways you could set that up on Twine so you'd have it run the character generation script and overwrite the old PC when you died.
Twine is the HTML-based software used for The Howling, St Miluina's, and similar games, not that hard to learn it but it also tends to end up fairly text-based and is certainly on the simpler style of game if you're not getting super intricate with the coding, like some of the Ren'Py visual novels.

Amalgamation viewtopic.php?f=79&t=57821&hilit=amalgamation
You have a companion you can eat or feed to someone else. Though the game hints at you that it may be a bad idea as she is important to the story.

Then there's vgame

You have two companions you can eat at any time. And they can not only be eaten by others in battle, I found it almost impossible to prevent.

I was thinking of making an RPG maker game with this very thing in mind. It'd be set in the Monster Hunter universe and you would just control hunters and go out on hunts, but people in your party could get eaten and killed instead of just carting. What makes it more interesting than just nameless mooks getting got is that any gear they have would either be destroyed or damaged and would need to be recovered and repaired. But with school and work and RPG Maker costing money I haven't gotten around to really digging deep into it.
