The new tool doesn't fully work for those who can reform (perhaps unless the function is fully turned off I imagine), so that would account for at least Grihm and Zagreus I imagine. Not fully sure about Rake, I don't remember him being able to reform in MMA, but I could be wrong. I've also had bug #3, and had it happen pretty consistently when using the push option in Macro/micro scenarios.

Rake, Grihm, Zagreus, and Mysti. I dunno who Grihm is based on off the top of my head, but I know Rake is based on Cloud from Final Fantasy, and Mysti is from another of Mysta's games I believe. Basically, these characters can revive in their own canons so they can revive in this game as well.
