Start with picking a tone and setting you like.
For example:
A fantasy game where you play as an assassin picking off prominent members of a royal court, if you want a setting where you easily can have a wide variety of different species as preds/prey and tie the vore into some deep lore, and a darker tone where getting eaten is the end.
A cozy dating game where you meet up with lots of diferent preds/preys, for a more down to earth setting where you dont HAVE to do too much worldbuilding to immerse the player, and a more light-hearted tone that can focus more on endo or reformation if you want some weight gain involved.
A game where you captain a pirate vessel and have to manage your provisions and your crew's satisfaction, launching raids and boardings to get loot and to feed, fits any setting from the Bronze age to sci-fi to fantasy so go ham, this would definately be on the lethal side, but could otherwise go any way tonaly. It could be a hyper tense balancing act, where one wrong move secures you an end padding your first-mate's waist or a swashbuckling adventure where the risks of mismanagement comes secound to banter with the crew.
If it's your first time making a game, I'd recommend a visual novel, if not go with whatever you feel most comfortable doing.
I hope I've helped give you a starting point with these ideas, and I hope you figure something out.
