Her first meal? I find it hard to believe she's never indulged before, and in such a case I applaud her strength of will for resisting so long!
Unless you mean her first meal of the day, in which case I rescind my previous applause :p

Yeah, I understand the confusion. This sketch is meant to be a potential pre-cursor to Shrabby's pic of Chrysalis digesting Fluttershy, while Twilight begs Chrysalis to let Fluttershy go. In that pic, Rarity is her "first" meal, and is already a pile on the ground.
