I have an idea for that. Draw the ribcage as if it had no sternum and it was stretching outwards. So like, on the side of the torso, it follows the curve up from the side, and then you can just place a stretched tit anywhere along that. And since it's bones it doesn't expand like skin so you can keep the top part well defined.
