Prior to getting dummy thicc, Ava had a small and petite figure. In order to fill out her curves, she swallowed an unsuspecting Cocoa depicted here.

Now, with her defenses down, Cocoa manages to escape from Ava, having somehow survived all this time inside her. Escaping with the surviving prey that Ava swallowed before.
