Leo is making sweet love to Sumi who is in her inflatable doll form before swallowing her whole, taking a moment to enjoy her inside him, before releasing her cleanly out of his bum... but Sumi has a surprise for him as she comes out as a Valentine's Heart Balloon, which Leo then hugs close.

The simple fact Sumi allows Leo to have fun with her in this state, coupled with him never taking advantage of that, is a statement of the sheer level of trust between the two as a couple.
