Imagine that you cant die, and you are buried under tons of earth, maybe you are inmortal but would not be a nice situation, they cant die, but they can feel pain, they can suffer, they can feel bad, for exameple if the preys are not digfested one time each day their bodies start to feel bad because the "toxins" accumulate. So they can feel danger, maybe no a "mortal" danger but danger in the end.

Apart from mental anguish what actually causes them pain? Because having their entire body destroyed (digestion), or having their head pierced and an object rooting around inside their brain seemed to cause them pleasure.

Also, how are they immortal?
The body needs to take in energy in order to move or even function, it's why we need to eat, eventually they would run out of energy and at the very least lose conciousness, unless they somehow absorb energy from the environment or the laws of physics work differently in this comic's universe.
