I think you could put even more risk and less certainty into a game like that! :D
Following the rules they picked, it was already clear one (and only one) would come out alive and they won't be seeing each other again, no matter which outcome. The "only" risk you're taking is which role will you be in.
What if, instead of a direct competition, they both roll against a fixed score? If one of them beats it and the other doesn't, the latter is lunch. But there's also a chance that noone makes it or both win and nothing happens (in which case you can always try again another day, maybe with a higher fixed score).
If I'm going first and win, I'll have to wait to find out whether I'm gonna keep a friend or gain some fat. If I lose, fate still has to decide whether I'm gonna feed him or we both get churned together by some lucky third party.
