Gengar is a Pokemon who loves being mpreg, and Rotum, the simp he is, makes sure he's mpreg as often as he can to share a closer bond with Gengar. The two often breed with each other, and while Rotum can't impregnate Gengar since he lacks the means to do so, what he can do is directly transfer his brood into Gengar since they're ghosts.

However, broods, whether eggs or live, aren't the only things he can carry in his womb. He can also extract the souls of his devoured victims (either orally or unbirthed) and imprison them in his womb just for the sake of keeping his belly nice and big for Gengar. Though if he gets bigger than Gengar at any point, he'll either transfer the souls into Gengar, digest them, or release them. It all depends on his mood at that moment in time.
