I think the rib problem only happened because Fiona started bending so high up, before she got all the way in. If the ribs are elastic and in tension, they're being pulled towards their anchor points on the spine. The more that the front of the rib cage rotates up and out, the more that force is going to be crushing the top and bottom ribs together and folding the sternum, which is probably unpleasant. If they rotated far enough, there would be a tipping point where it would be easier for the bottom ribs to resolve that tension by pulling in OVER the top ribs, essentially flipping the front of the ribcage inside-out. Fiona didn't go that far, but she did go far enough to cause Emma pain.
Now that Fiona's all the way inside, she's less constrained by the ribs and more by the walls of the stomach/abdomen. That part appears to be softer and more flexible.
