The way I do it is you find a model with at pregnancy slider, scale down everything but the belly, and then scale up the pelvis to make it seem like a big bulging belly. You then simply position it in front of the model you want to be the pred. For my bellies, I personally use Piper Wright (https://sfmlab.com/item/2277/) as she has a large pregnancy slider and is easy to work with.
