Mae is getting BVd by Hereith, head first, in such a manner that Mae's cooch is now right in Hereith's face. Hereith is therefore, taking the chance to eat Mae out.

The confusion probably comes from the fact that not everyone knows hereith has 4 arms and the way hers look in the picture. My first thought was that mae was fondling hereith's breasts....... somehow... but after I learned who was eating who and 'how', the image came together quite nicely
